Do I have this correct? Residents have limited work hours, almost certainly diminishing how much they can learn, but some program directors think it's OK for them to use their non-work time to be actors in a reality show? It's OK to learn to be an actor but not learn more surgery? Really? What's wrong with this picture?

Sorry about the delay in responding. Since it's a reality show, I assume they would be shooting a lot of the content at the hospital. It's not only time-consuming, it's a distraction from the primary purpose of the hospital and residents which of course is the patients.
